• Completed loan application (All legal owners of subject property are required to be included on the application.)
• Signed disclosure regarding home equity and title insurance
• Signed fixed rate disclosure statement
• Copy of your deed
• Copy of most recent paid tax receipts for school and property taxes or year end mortgage statement if taxes are escrowed.
• Signed checklist

Additional items that may be required:
Copy of existing title insurance policy or abstract of title only if title insurance is required.
